Preparing for Your Visit
Please help us ensure a smooth and efficient visit by bringing the following items:
- Referral slip and any recent X-rays from your general dentist
- A list of current medications you are taking
- Insurance information, including cards or forms
- Completed online patient registration and medical history forms (available on our website)
Please note: Patients under 18 must be accompanied by a parent or legal guardian.
If you have any medical conditions—such as artificial joints or valves, heart murmurs that require pre-medication, severe diabetes, hypertension, or any other concerns—please notify our office prior to your appointment.
